DR. Dorothy Delisfort-Crisostomo, DNP, FNP-C, PMHNP-BC

Dr. Dorothy Crisostomo is both a Board-Certified Mental Health and Family Nurse Practitioner. She completed her Doctorate Degree in Nursing Practice at Brandman University in Irvine, California. She completed her master’s degree at Walden University, Baltimore, Maryland in Family Nurse Practitioner and her bachelor’s degree at Georgia Baptist College of Nursing of Mercer University in Atlanta, Georgia. She has a strong background in family medicine, mental health and outpatient healthcare. She resides in Gwinnett County with her family and her two children. She believes in developing strong relationships with patients in order to provide a personalized level of care, tailored to each patient’s specific needs. She is a dedicated, patient-focused professional with proven strengths in acute patient care, staff development and advocacy. She is self-directed and develops educational programs for many world-renowned hospitals.. She possesses strong analytical skills and has a proven record of reliability with demonstrated leadership abilities.

She was President Elect of AMSN from 2006 to 2008. She is a member of the following organizations: AANPCP, ANCC, ANA, Georgia Hospital Association, American Nurses Association, Georgia Organization of Nurse Leaders, Board Member of NPC, AMSN, Sigma Theta Tau, Nurses Changing Community and Holistic Nursing.

She served as clinical instructor at Georgia State University and is the CEO for U-First Healthcare. She is a Co-Founder and Member current Chairwoman of The Haitian American Nurses Association of Georgia. She also serves as the Treasurer of The United Advanced Practice Registered Nurse of Gwinnett and Forsyth.

She is also a member of The International Women of Hope, GA Haitian-American Chamber of Commerce and the Managing Partner at Final Touch Entertainment. She is actively involved with helping nursing students embrace compassion and self-care as they progress through nursing school. She is an active mentor to a wide variety of health care students focusing on mental health, primary care, health care access, addiction medicine and whole person care. Additionally, she has been a speaker locally on a variety of topics including clinician self-care, holistic nursing, various health topics, faculty development and integrative medicine and health.

She enjoys spending time with her family and friends, being in nature, running, playing cards, dancing, reading, learning and teaching.
